Registration Forms

For all new clients who have upcoming appointments with Clinical Psychology Services, we have a convenient way for you to download all registration forms prior to initial visit.  We understand time is precious and this way you will not have to come in earlier or take up time from the full consultation visit.  The registration is under an Adobe Acrobat PDF file and can be accessed on any computer.  Included forms are Registration, Informed Consent, Financial Policy, and Privacy Notice.  Please download these, print and fill out all necessary information, and bring to the first visit.
